PUPPY SILVER WELLNESS PLAN

The PUPPY SILVER WELLNESS PLAN is a package of services covering the puppy’s first year of life.  It helps provide the best possible care for your pet at an affordable price.  Examinations are included until 6 months of age and 50% off from 6-12 months of age.

Services included:

·     Initial Complete Examination      

·     Vaccinations  Three   DA2P-P   

§ Two Corona       

§ One Bordatella        

§ One Rabies     

§ Two Giardia 

·     Fecal examinations (two)   

·     Frontline or advantage (two)   

·     Deworming (four) Strongid (Drontal extra)

     The above procedures are normally done at two-week intervals starting at 8 weeks of age.  Vaccines are given at 3-4 week intervals. Nail trims are included.  Ovariohysterectomy (Spay) Neuter – by surgical laser, is included and required  to be done between 5-6 months of age and before reaching 50 lbs body weight.   (If the pet is in heat, pregnant or over 6 months there will be an added fee, if over 50 lbs, $3.50 per lb over 50 will be charged).  Pre-anesthetic blood tests, pain medication, IV Fluids and pulse oximetry monitoring are included.

Costs: Total fees - $515.00 ( $315.00 without the spay/neuter surgery), plus $6.00 enrollment fee

Payment options:

1.   Pay in full, we accept cash, checks, debit cards, and all major credit cards.

2.   Pay 1/3:     Spay/Neuter Included - $177.66  at first visit, then $171.67 at next two visits.

W/O Spay/Neuter – $111.00 at first visit, then $105.00 at next two visits.

Conversion to Adult Basic Plan:

When your pet turns one year old this plan will expire and your pet would be due for a complete examination, at that time we can discuss the CANINE GOLD WELLNESS PLAN.    We also highly recommend a microchip to identify your pet.  This can be done for $37.00 (regular $47.75) if you buy this plan. If this plan is accepted and later cancelled prior to the expiration date, you will be responsible for the full amount of all services received.

Other Services:

If  your pet becomes ill or injured,  treatments, diagnostics and medications are discounted by 10% with the exception of Pet Food.  Boarding is discounted by 15%.
