CANINE Platinum WELLNESS PLAN

The CANINE PLATINUM WELLNESS PLAN is a package of services that Your Pet needs on a yearly basis.  It naturally follows the CANINE GOLD WELLNESS PLAN with expanded services for a pet with greater needs.

Services covered are:

·         Complete Physical Examination, twice yearly

·         Anesthesia and Dental Prophy (StageI-III)once yearly

·         Nail Trims/Anal glands (as needed)

·         Routine Vaccinations:

·         Bordetella (Kennel Cough)

·         Coronavirus           

·         DA2P-P (Distemper-Parvo Combo)   

·         Giardia (booster included if needed)  

·         Rabies (every 3 years) 

·         Laboratory Tests:

·         Complete Urinalysis (Twice yearly) 

·         Fecal Examination (Twice yearly)   

·         Thyroid Level                   

·         Complete Profile/CBC   

·         Parasite Control: Deworming (max 2 large dog drontal, 1X yearly)      1 Flea treatment

·         Counseling: as needed or twice yearly for small dog

·         Nutrition                                                                                                          

·         Behavior                                                                                                         

If Your Pet becomes ill or injured while on the CANINE PLATINUM WELLNESS PLAN, diagnostics, treatments, and medications are done at a 10% discount. Boarding is provided at any time while on the plan for a 15% discount.  We also highly recommend a microchip to identify your pet.  This can be done for $35.00(regular $45.90) if you buy this plan.  If this plan is accepted and later cancelled prior to the expiration date, you will be responsible for all services received at the full price. 

Cost:  $499.30, plus $6.00 enrollment fee

1.      Pay in full, we accept cash, checks, debit cards, and all major credit cards.

2.      Pay 1/3:  $172.44 (includes enrollment fee) now, then $166.43 in 30 days, then $166.43 in 60 days.
